Robert Alan Benson, license no. CA011209L of Pittsburgh, Allegheny County, was
ordered to pay a civil penalty of $300 and, within six months, complete 12 hours of
continuing professional education because he did not complete the requisite 80 credit

Marc Silverstein, certificate no. CA047082 of Holland, Bucks County, was assessed a
$10,000 civil penalty for practicing public accounting without a current license.

David Thomas Shergalis, certificate no. CA011542L of Media, Delaware County, had
his certificate of certified public accountant and current license to practice public
accounting suspended for four days for his late payment of a previously imposed civil

James Patrick Flanagan, license no. CA020184L of Drexel Hill, Delaware County,
was ordered to pay a civil penalty of $2,000 because he engaged in the practice of
public accountancy and used the designation “Certified Public Accountant” and the

Lackawanna County
Bonita & Rainey, license no. AF000729L of Plains, Lackawanna County, was
ordered to pay a civil penalty of $5,000 because it practiced as a certified public
accountancy firm when it was not currently licensed and registered between Jan. 1,

Washington County
Adam C. Garczewski, license no. CA041640L of Lawrence, Washington County, was
ordered to pay a civil penalty of $300 and, within six months, complete 21 hours of
continuing professional education because he failed to complete at least 80 hours of
continuing education during the relevant reporting period from Aug. 2, 2008, through

Westmoreland County
Mark A. Terdle, license no. CA022325L of North Huntingdon, Westmoreland
County, was ordered to pay a civil penalty of $900, had a formal reprimand placed on
his licensure record, and must, within six months, complete 40 hours of continuing
professional education because he did not complete the requisite eight hours of
continuing education in tax subjects during the biennial period, did not complete the
requisite 20 hours of continuing education during calendar year 2008 in the biennial
period, and did not complete the requisite 80 hours of continuing education during the

Cara M. Ihndris, license no. CA052118 of Rockville, Md., was ordered to pay a civil
penalty of $600 and, within six months, complete 16 hours of continuing professional
education because she did not complete the requisite 16 hours of continuing education
in accounting and auditing subjects during the biennial period and did not complete
the requisite 80 hours of continuing education during the applicable biennial period.

PA Board of Certified Real Estate Appraisers
York County
Robert C. Stoner, license no. RL003684 of Dillsburg, York County, had his license
suspended for at least 15 months, with the suspension stayed in favor of probation
after a minimum of three months, subject to terms and conditions, because he failed
to complete an appraisal assignment within the requirements of the Uniform
Standards of Professional Appraisal Practice; failed or refused to exercise reasonable
diligence in developing an appraisal, preparing an appraisal report or communicating
an appraisal; and negligently or incompetently developed an appraisal, prepared an
appraisal report, or communicated an appraisal. He was also ordered to pay a civil
penalty of $3,000, pay costs of investigation in the amount of $1,000, and complete at

Construction Junction Inc., of Pittsburgh, Allegheny County, was ordered to pay an
administrative fine of $2,000 for violating the Solicitation of Funds for Charitable
Purposes Act by failing to keep true and accurate records. (05/10/11)
Indiana County
Indiana Lions Club, of Indiana, Indiana County, was ordered to pay an administrative
fine of $4,000 for violating the Solicitation of Funds for Charitable Purposes Act by
making material false statements on applications, statements, or reports required to be
filed with the Bureau of Charitable Organizations for fiscal years ending June 30,

Dolce Suono Ensemble Inc., of Philadelphia, Philadelphia County, was ordered to
pay an administrative fine of $1,500 for violating the Solicitation of Funds for
Charitable Purposes Act by soliciting charitable contributions in Pennsylvania

Schultz & Williams Inc., of Philadelphia, Philadelphia County, was ordered to pay an
administrative fine of $1,000 for violating the Solicitation of Funds for Charitable
Purposes Act by failing to file a contract with the Bureau of Charitable Organization

Nexus Direct LLC, of Virginia Beach, Va., was ordered to pay an administrative fine
of $3,000 for violating the Solicitation of Funds for Charitable Purposes Act by
providing professional solicitor services prior to being registered with the Bureau of
Charitable Organizations and by failing to file a contract with the bureau prior to

Nellie R. Wortman, license no. DC000900L of State College, Centre County, was
assessed a $1,000 civil penalty for practicing chiropractic without a current license.

to pay a civil penalty of $2,300 because it practiced nail technology in a grossly
incompetent or unethical manner through the use of razor tools and razor blades,
failed to maintain tools that come into contact with a client in a sanitary condition at

technology in a grossly incompetent manner by having five razor scraper tools and 70

Vu Thanh Nguyen, t/d/b/a Tina Nails, license no. CY106565 of Folcroft, Delaware
County, was assessed a $2,900 civil penalty for having practiced in a grossly
incompetent or unethical manner by having a hot wax bowl and instruments used for
facial waxing, multiple razor scraper tools at workstations, no wet sterilizer or
container for sanitized tools, dirty manicure tools at workstations and paper towels

All in Wrist, license no. CB121341 of Darby, Delaware County, was assessed a
$1,950 civil penalty for engaging in booth rental, maintaining the salon in an
unsanitary condition and failing to have a complete first aid kit. (03/17/11)

Salon 1 Style Inc., t/d/b/a Salon 1 Style, license no. CB121933 of Lancaster,
Lancaster County, was assessed a $1,550 civil penalty for engaging in booth rental,
maintaining the salon in an unsanitary condition and failing to have a complete first

Robert M. Licata, license no. CO150052L of West Pittston, Luzerne County, was
ordered to pay a civil penalty of $1,000 because he practiced cosmetology while his

Glamor Nail Spa LLC, t/d/b/a Glamor Nail Spa, license no. CY195490 of
Tannersville, Monroe County, was ordered to pay a civil penalty of $1,000 because
Glamor Nail Spa LLC, while trading and doing business as Glamor Nail Spa,
permitted an unlicensed individual to perform cosmetology services in the salon and
maintained an unlicensed cosmetology shop by providing waxing services to a client

Philadelphia County, had her license suspended for failing to pay a previously

Philadelphia, Philadelphia County, was ordered to pay a civil penalty of $500 because
she employed an unlicensed person to practice cosmetology and aided and abetted the

Anthony J. Pichini, license no. DS014935L of Mt. Penn, Berks County, voluntarily
surrendered his dental license because he practiced dentistry from April 1, 1999, until
approximately September 2010, and failed to renew his license during said period.

Thomas W. McFarland Jr., license no. DS020097L of Wynnewood, Montgomery
County, was ordered to pay a civil penalty of $5,000 and his license remains
indefinitely suspended under the terms of a previous board order because he engaged
in unprofessional conduct by failing to follow current infection-control
recommendations issued by the Centers for Disease Control and Prevention.

Martin Jay Thaler, license no. DS022305L of Monsey, N.Y., received a public
reprimand and was assessed a $1,000 civil penalty based on disciplinary action taken
against his license by the proper licensing authority of another state and having failed

Patricia A. Sabers, license no. DS023465L of Sarasota, Fla., received a public
reprimand and was assessed a $1,000 civil penalty based on disciplinary action taken

revoked based on disciplinary action taken against his license by the proper licensing

Gary Scheiner, unlicensed of Wynnewood, Montgomery County, was ordered to pay
a civil penalty of $1,000 and to immediately cease and desist from engaging in the
practice of medicine or attempting to offer to practice medicine in Pennsylvania

felony or a lesser offense incompatible with the duties of a notary public and was not

Cumberland County
Alan F. Spurgin, of Lemoyne, Cumberland County, had his notary commission
suspended for a period of three months and was ordered to pay a civil penalty of $200
because he failed to keep and maintain custody and control of an accurate
chronological register of all official acts done by virtue of the notary’s office.

Brenda J. Barto, of Jonestown, Lebanon County, had her notary commission
suspended for six months, with three months of active suspension and the remaining
period stayed in favor of three months probation; was ordered to pay a civil penalty of
$500 and $150 for the costs of continued probationary monitoring; and was required
to attend a notary education practice and procedure course because she failed to know
through personal knowledge or have satisfactory evidence that the person appearing
before her was the person described in and who was executing the instrument and she

Tracy Talpas Wallace, license no. RN349603L of Oakdale, Allegheny County, had
her license suspended for three years, retroactive to Feb. 24, 2010, with one year of
active suspension and the remaining two years stayed in favor of probation upon
compliance with continuing education regulations, based on findings that she violated
the terms and conditions of her Voluntary Recovery Program agreement. (03/18/11)
William Charles Minneker, license no. PN279941 of Carnegie, Allegheny County,
had his license suspended, effective immediately, pursuant to the order of the Court
of Common Pleas of Allegheny County dated Jan. 3, 2011, which the court issued

her license indefinitely suspended for no less than five years, with the suspension
stayed in favor of no less than five years of probation in the Disciplinary Monitoring

nursing with reasonable skill and safety to patients by reason of mental or physical
illness or condition or physiological or psychological dependence upon alcohol.
